The numeric rating scale (NRS) is the most common way to measure pain intensity. We use it at assessment and at every follow-up.

0 means no pain and 10 the worst pain you can imagine. This is the scale we use during assessment.

What does the score mean?
0
No painYou feel no pain at all.
1–3
MildNoticeable, but you can do daily tasks comfortably.
4–6
ModerateDistracting; may affect your sleep and work.
7–10
SevereClearly limits daily life; rest doesn't relieve it.
How to keep a pain diary

Twice a day
Note your pain score at the same times each morning and evening.

Triggers
Write down movements and positions that make pain better or worse.

Medication
Record when you take painkillers and how well they work.
